中一入學申請表    Application for Secondary 1 Admission


申請程序 Application Procedure：
步驟(一)
 Step (1) 		申請人必須首先填妥網上申請表格 Applicant must first complete the online application form
　 a. 申請學生的個人資料 Personal particulars of the applicant
b. 曾／就讀學校名稱 Name of school(s) attended / attending
c. 專長和興趣 (包括體育、社會服務、藝術等) Talents and interests (including sports, community services, art, etc.)
d. 報讀本校的原因 Reasons for choosing our school
e. 家庭成員資料 Family information
f. 曾／正在中華基督教青年會小學／中學就讀的兄弟姐妹資料
   Information on siblings who are studying / have studied in Chinese Y.M.C.A. Primary/Secondary Schools
備註 : 申請人必須提供電郵地址，以便獲取有關面試或其他資訊。
Remarks: Applicant must provide email address for acquiring information about interview arrangement or other details.
 
 
步驟(二)
 Step (2)
 
 		 申請人完成網上申請表格後，便可遞交表格 (按「提交」)，系統便會自動提供一個「申請號碼」，以作日後溝通之用。
After completing the online application form, applicant can submit the form (click "Submit") and an "Application Number" will be provided automatically for future communication.
備註: 遞交申請表格前，請檢查清楚所有資料是否齊備。申請人確認資料後 (按「提交」)，所有資料均不能更改。如發現資料不正確或有遺漏，該申請將不予處理。
Remarks: Please ensure all information is completed before submitting the application form. Once the applicant has confirmed the information
               (click "Submit"),it cannot be changed. If any information is found to be incorrect or missing, the application will not be processed.

 		 請把已填妥的網上申請表格列印出來 (附有申請號碼)，貼上相片，並在家長/監護人位置簽署，連同所須證明文件之影印副本郵寄/遞交至 : 新界 天水圍 天富苑 中華基督教青年會中學。
Please print out the completed online application form (with indicating the application number), paste the student’s recent photo, sign it with the applicant’s parent / guardian's signature, and mail / submit it together with photocopies of the required supporting documents to the Chinese Y.M.C.A. Secondary School, Tin Fu Court, Tin Shui Wai, N.T.
 
 
步驟(四)
 Step (4) 		遞交申請表時，須繳付 HK$75 報名費（可選擇以下其中一種繳費形式）：
A non-refundable application fee of HK$75 is payable upon submission of the application form (please choose one of the following payment methods).
　 a. 現金
    Cash
b. 劃線支票（抬頭請書寫「中華基督教青年會中學」）支票背面須寫上申請學生姓名及聯絡電話
    Crossed cheque (payable to "Chinese Y.M.C.A. Secondary School") with the applicant's name and contact number at the back of the cheque
c. 直接轉賬於本校戶口 ，並將入數紙副本交予本校存檔
    Direct transfer to our school account and submit a copy of the bank-in slip to our school for record
 
 
步驟(五)
 Step (5)
 		 申請程序完成後，申請人便會獲發申請費收據，以供記錄。
Upon completion of the application process, applicant will be given a receipt of the application fee for record purpose.
